Application Procedure

Procedures and notes of application for the
“Certificate of Case Reporting Record” in person

I. Documents to submit
  • Completed application form(Note.3); Download application form
  • Copy of the applicant’s identity document with the original presented for verification(Note.4 e 5);
  • If it is necessary to file a claim with an insurance company, a copy of the insurance policy must be submitted;
  • If the application is made on behalf of a company, an authorization letter with the company stamp(Note.6) and a copy of the authorizing person’s identity document must be submitted.
  • Note 1: Documents mentioned in items 2 to 4 above can be submitted afterwards (in person, authorizing others or by mail).
  • Note 2: If the supplementary document(s) is (are) to be submitted by mail, please send to “Administrative Centre, Judiciary Police Building, No. 823, Avenida de Amizade, Macau”.
  • Note 3: Application forms can be obtained by downloading from the Judiciary Police website or collecting from the Judiciary Police in person. The application form must be completed and duly signed with the purpose of application stated by the applicant.
  • Note 4: If the identity document is stolen, a copy of travel document, birth certificate or other valid identification documents can be submitted instead.
  • Note 5: Application by a minor must be filed by his/her legal representative (parent or guardian). The application form must be signed by the legal representative and submitted with the copies of the legal representative’s as well as the minor’s identity documents.
  • Note:6:
    a) If a company employee (the complainant) is authorized to apply for the “Certificate of Case Reporting Record”, his/her testimony must first be recorded in the Judiciary Police;
    b) If the complainant and the authorized person are not the same person, the authorized person must have his/her testimony recorded in the Judiciary Police first in order to confirm the loss of the company reported by the complainant;
    c) If the employer / owner / person in charge of the company would like to apply for a “Certificate of Case Reporting Record” on his/her own, it is also necessary to record his/her testimony in the Judiciary Police first in order to confirm the loss of the company reported by the complainant.
  • Note.7: The “Certificate of Case Reporting Record” would be issued within 10 working days after all the required documents are received.

* As the “Certificate of Case Reporting Record” involves important personal information and confidential criminal case information, the applicant / interested party or authorized person must keep the related information confidential.

II. Fees
  • Fees:
    Calculation based on provisions of Stamp Duty General Table item 11:
    Each page: MOP 5
    Each certificate: Add MOP 10.
  • Postal fee(Double registered mail) (If postal service is required):
    Local───MOP 27(Fee for double registered mail) + Postage (Note)
    Non-local──MOP 34(Fee for double registered mail) + Postage(Note)

  • Note: Please refer to the Table of postage tariff announced by the Macao Post for postage. The applicant may choose either surface mail or air mail but tariffs for surface mail, air mail and different regions are different.

  • Special situations:
    1. If the applicant is unable to pay the fee when making the application (e.g. applying after being robbed or other situations), the payment can be made when collecting the certificate in person or by an authorized person. (Authorization Letter).
    2. If the applicant is located overseas, an email can be sent to the Case Reporting and Emergency Operation Centre of the Judiciary Police: piquete.sede@pj.gov.mo for enquiring the application procedure and payment methods.
III. Time and location of application
  • Office hours:
    Monday to Thursday : 09:00-13:00 / 14:30-17 :45;
    Friday: 09:00-13:00 / 14:30-17:30
  • Location of collection:

    Cotai Sub-bureau – Case Reporting and Emergency Operation Centre: The Judiciary Police Cotai Sub-bureau Building, Estrada Flor de Lótus
